Hi everyone,
We are a small non-for-profit organisation that have launched our Connected to Care Registered NDIS Provider service recently. We offer support coordination primarily as well as specialist support coordination, SLES, Community Participation, and Support Work. We started our journey with the NDIS to reach our community in this way after having some negative experiences with other providers through clients gained from our grant funded care coordination program. We also wanted to put our expertise in mental health to good use in this space. Exited to connect and learn from everyone!
